Passage Into Time

This is a very interesting book and a bit eerie. It seems "Out-worldly" to lose technical equipment such as airplanes in a perfectly clear sky, especially when that's not what the pilots report seeing. Ships that disappear within hailing distance of the safe shores is also spooky. The book is really worth taking the time to read and get into. The photographs also are a great help.

A must read for those who love the Bermuda Triangle.

Gian Quasar's book is the best new book on the Triangle mystery since the 1970's.The book is well documented and updated with more recent vanishings and weird happenings! It is a pleasant read,with a fresh perspective on the subject and some new theories.
Mr.Quasar has researched this subject for many years.He isn't just an armchair adventurer.He has visited the places of the vanishings.He has literally sailed and flown the same paths of those who have vanished. Like a detective,Mr.Quasar will continue to investigate until he solves the mystery. I have read this book cover to cover several times since i purchased it.Very enjoyable. Can't wait until the movie !!

Fascinating Read!

This book presents a fascinating account of the riddles of the Bermuda Triangle and explores possible explanations. Bermuda Triange is well known not only for missing planes and vanished ships but also UFOs and sunken worlds.

Areas in the Bahamas were reported to hold the remains of a vast prehistoric civilization that sank beneath the sea. UFOs and USOs were also frequently reported in the Binini area and other areas of the Bermuda Triangle.

In particular, the stories of those who lived to tell are baffling. There are unexplained forces involving electronic drains, radio dead spots, power blackouts, compass malfunction, unusual fogs or flares. Countless theories have been proposed to explain the many disapperances within the Bermuda Triange. The most practical one for vanished ships seems to be environmental (e.g. unusual and extremely sudden winds or volatile weather) and human error. Persistence of unexplained electromagnetic field anomalies at places of most disappearances may suggest transient magnetic vortices there. Ships and planes traveling through or near these vortices might simply disappear by being held in time!

About the Author

MEA CULPA BY A BIG PHARMA CEO

Pfizer's CEO, Dr. Hank McKinnell has written an astonishing book in which he admits that he doesn't always believe in what he's saying [11], that drugs from Canadian pharmacies are safe [69] and that high US drug prices have nothing to do with past R&D expenses [46]. He also writes that "perhaps pharmaceuticals represent too low a percentage of total healthcare spending" [45] and he calls for "price controls to be lifted" around the world [64], because "It is time for Canadians and others to pay their fair share." [65]. He also calls for a doubling of drug patent life [185] which would result in a drastic reduction of new, low-priced generic drugs.

Dr. McKinnell starts his book with the surprising confession that he doesn't always believe in what he's saying. "They listened to my logic, but I could tell they weren't convinced, and to tell you the truth, I wasn't either." [11]

He also doesn't shy away from embarrassing facts, "Branded drug prices are anywhere from 25-100 percent more expensive in the United States." [50] He even admits, "Drugs from Canadian pharmacies are as safe as drugs from pharmacies in the United States." [69]

But his impressive mea culpa doesn't stop there. He slams everyone who makes a connection between drug prices and R&D. "It's a fallacy to suggest that our industry, or any industry, prices a product to recapture the R&D budget spent in development." [46]

Finally, in an astonishing intellectual somersault, Dr. McKinnell claims that "price controls always make prices higher in the long run." [64] And since he wants to give people lower drug prices, by eliminating price controls, he writes, "Starting with pharmaceuticals, I call for price controls to be lifted in Canada and elsewhere." [64]

Dr. McKinnell ends his book with a wonderful quote by Gandhi, for those who desire change. "First they ignore you. Then they laugh at you. Then they fight you. Then you win." [193] Dr. McKinnell just doesn't realize that he has become "them."

Big Pharma's Mea Non Culpa

If you buy the book "A Call to Action" by Pfizer's CEO, Dr. Hank McKinnell, with the notion that you will learn what this pharma executive believes the industry should do to reform itself, you are in for a big surprise. Notwithstanding an Amazon.com review of the book written by Peter Rost -- Pfizer's whistleblowing, soon-to-be non-head of Pfizer's endocrine care marketing unit and "Pharma's Black Knight" -- in which McKinnell is described as making "an impressive mea culpa," this book is actually a mea non culpa!

I had reason to expect to see suggestions for changes in the way pharma operates because at the very beginning of the book (page 13), McKinnell said that "if Pfizer wanted a seat at the table where important healthcare solutions were being hammered out, we had to earn it by acting differently, partnering differently, and communicating differently." Unfortunately, the rest of the book does NOT carry through on this theme and focuses instead on how consumers, society, employers, managed care, and government need to act differently.

Before getting to the action items, McKinnell tries to answer the most important questions put to the industry by critics and the public. These questions are:

* Why are Prescription Medicines So Expensive?
* Why Does the Industry Do So Much Advertising?
* Why Do Americans Pay More Than Canadians for Drugs?

Unfortunately, McKinnell sticks to the standard pharma industry playbook except on one or two occasions where he seems to offer contrary opinions.

Why are Prescription medicines So Expensive? It turns out, according to McKinell, that drugs are not as expensive as you think (silly you!) and that branded drugs are cheaper than the alternative - the old "an ounce of prevention is worth more than a pound of cure" defense.

In essence, his argument is, drugs are not as expensive as you think, cheaper alternatives are soon available, and drug profits are good and well deserved.

I should acknowledge at least one refreshingly contrary view expressed by McKinnell: an admission that it is a "fallacy to suggest that our industry...prices a product to recapture the R&D budget spent in development." He suggests instead that it is "investors' confidence in the risk and rewards" that drives prices.

McKinnell takes exception to critics such as Senator Bill Frist who claim that "increased direct-to-consumer advertising has helped fuel escalating drug costs." McKinell, like PhRMA, insists that "studies show there's no correlation between prices or price increases and DTC advertising."

McKinnell cites a study done in 2001 by The National Institute for Health Care Management entitled "Prescription Drug Expenditures in 2000: The Upward Trend Continues." McKinnell claims that this study shows that the "average price of drugs that are advertised is lower than those that aren't." The report, however, concludes that the increase in prescription drug spending is due, in part, to "more aggressive marketing of prescription drugs to both doctors and consumers."

Why Does the Industry Do So Much Advertising? McKinnell wants us to believe that DTC advertising is really education (he prefers ro call it "DTC education"). This is not a new mantra. Johnson & Johnson Chief Executive William Weldon, for example, said in an address to PhRMA: "Our communication with patients should really be thought of as Direct to Consumer Education."

Why Do Americans Pay More Than Canadians for Drugs? To be honest, I didn't read this chapter. According to Peter Rost, "McKinnell ... admits that drugs from Canadian pharmacies are safe ...[and] that 'perhaps pharmaceuticals represent too low a percentage of total healthcare spending' and he calls for 'price controls to be lifted' around the world, because "It is time for Canadians and others to pay their fair share."

Chapter 13 is the chocolate nugget of the book. In this chapter McKinnell lists 10 actions that he believes should be taken to reform the healthcare system in the U.S. Only one action on the list, Reform Direct-to-Consumer Advertising, is an action item for the drug industry. All the others are the responsibility of individuals, society, employers, managed care, and governments, not the pharmaceutical industry. Coincidentally, at least six actions would either benefit the pharmaceutical industry in general or highlight specific Pfizer initiatives. Rather than focus on these - you can read more about it in the Summer 2005 issue of Pharma Marketing News - I'd like to focus on Mickinnell's suggestions for reforming DTC.

Reform Direct-to-Consumer Advertising
This is the one action item mentioned in the book that applies directly to the pharmaceutical industry and can be said to require a change in the way it does business. Here's what McKinnel recommends for DTC:

* Improve communications about risk
* Provide information about alternative therapies available, including therapies that do not require a medicine
* Promote prevention and wellness, compliance with treatment regimens, and disease awareness
* Promote access to medicines and do a better job communicating about patient assistance programs
* Implement external oversight of DTC, including working with the FDA to get their views on advertisements before they are run

Mckinnell's action plan for DTC reform, however, does not go as far as some other emerging principles do. He does not suggest, for example, a 1-year moratorium on DTC for news drugs or to limit TV ads to the appropriate audience at the appropriate time of day (something that Congress may impose on ED ads). Both of these principles have been adopted by Bristol-Myers Squibb in its Direct-to-Consumer Communications Code. It should be noted, however, that McKinnell has said in another venue "No erectile dysfunction drug ads on television except for 10 pm to 6 am. I'm in favor of that." (Washingtonpost.com Viewpoint interview, 2004).

Needless to say, this book is not going to shake up the pharmaceutical industry nor does it offer much new defense against critics of the industry. It often sounds like an advertorial for Pfizer's programs. A Pfizer spokesman referred a reporter to McKinnell's book as the "best source" for the company's perspective on advertising. I wouldn't recommend it, however, as the best source for an action plan to reform drug advertising in general.

A Decent Book Raising Legitimate Questions

Like it or not, to discuss about the healthcare system this book raised some legitimate questions such as sick-care vs prevention-care, free riding on the global market, malpractice law suits, third party payment system, etc.

There are some industry and Pfizer PR stuffs in the book. Anyway, you cannot expect a current CEO of a company to suggest something that would hurt his company or industry. (By the way, Peter Rost seems to be an interesting exception. When he does this, you need to do some research on history and ask why.) Still Hank McKinnell discussed seriously about the overall structural loopholes and possible reforms of the system.

Don't simply blame the pharmaceutical industry as the only "bad" boy in the game. It's true that not everything the companies do seems "right" under the bright sunshine, but who does in capitalism? That's why the system is designed to have balances and checks, while money chasing is the ultimate incentive in the market and real life.

If you know fair amount of biomedical science, you understand how little we know and how difficult it is to develop a novel and really effective and safe treatment under current knowledge. Healthcare as a whole is a very large and complex system. Because of the sensitivity the system deals with, which is the length and quality of human lives, it is already under the toughest regulations among all industries.

If you are open to new ideas and want to seriously think about the healthcare, you may find this book useful to provide some perspectives.

Objective & structured study of culture

The greatest value that comes out of this book is the ability to articulate exactly what we mean by culture, what are its various dimensions, & how a culture is manifest at the macro or national level- while stressing every now & then to separate a national culture from the individual value system of its citizens.

The first seven chapters elucidate on the meaning of culture, its dimensions, & the comparative scores of various different countries in each of these dimensions - what these scores mean in terms of the family, school, workplace, the state, & ideas. This consistent structure gives the reader a very nuanced point of view on different cultures & the way they manifest. In the latter chapters, Hofstede also considers scores on two cultural dimensions & explains the combined effect of these as a whole.

Chapter 8 does a great job of differentiating Organizational cultures from National ones & clearly articulates that values do not differ across organizations in the same country; what differs are the practices these values translate into. The book did not have as extensive research on Organizational cultures, but manages to establish what is, or should be, understood by Organizational cultures. And it also has interesting notes on how some of these Organizational culture dimensions corelate with national culture dimensions.

The final two chapters are notes on implications of these differences & the elements of culture shock & how to go about handling these at the level of family, school, etc.

So while this was one of the great books I read this year, I still think that the title of book is misleading - for organizational cultures is not the mainstay of the book; national culture is.

But Hofstede's very organized & structured prose adds a great deal to a very intangible soft topic like culture. I think I understand the world a little more having read this book & can try to differentiate the broad national culture traits in the people I work with from their own individual value systems.

Monumental Book Well Worth the Read

The father and son team of Geert and Geert Jan Hofstede have done a remarkable job breaking down the (measurable) elements of the world's cultures, usingt the somewhat antiquated IBM studies combined with more recent (less comprehensive) studies. The end result is that nations can be evaulauted on the basis of criteria such as "uncertainty avoidance," "individualism" and "power distance from superiors."

The work is enlightening and helpful to anyone who works internationally. It is also useful to break down one's own nation (for example, some Americans lean toward the British way of thinking while others are more German-like). The same criteria that divide nations also divide families within a society.

Businessmen, missionaries, pastors, counselors, journalists, and social scientists should devour these materials!

This should be required reading for anyone planning to live overseas or anyone who deals with internationals. In short, this book is relevant to our modern "shrinking" world and quite well done.

Like most significant works, this volume has its weak points.

Although the authors claim to espouse a "values neutral" position (which I have always argued is an impossible and illogical position), their Dutch/Swedish preferences ring out loudly and clearly (humanistic, environmnetalist, etc.). Although the authors do make a serious attempt to look at things from other perspectives, they simply cannot divorce themselves from their own cultural preferances. This is not bad -- they simply need to be above board and stop pretending to take the role of the neutral outsider (at least to better influence those of us who are American conservatives; we are big into distinguishing between fact and evaluation of fact; these evaluations are always done through a person's own personal gridwork).

The authors also have occasional trouble connecting a few dots. For example, on the bottom of p. 355, the Hofstedes are tactfully scolding the U.S. for its lack of foreign aid (again, showing their own bias), but on the top of p. 356 they add, "Looking back to half a century of development assistance, most observers agree that the effectiveness of much of the spending has been dismal." They then say those countries which did improve did so because of their cultural values, not foreign aid. But they seem incapable of concluding that good intentions (and even money) is not the most effective way to solve these problems. They just don't get it.

The same is true with contributions through governments to Tsunami relief. It should be expected that individualistic countries would be more prone to give as individuals, not as collective societies. Rather than look at total giving (or perecentage) OF A SOCIETY, they authors confuse a society with its government. Lots of missed "dot connections" in this work.

Despite the books weakspots, it is overwhelming strong and rich with fascinating content. It is a "mind opening" work -- well worth the read. You simply must read this one!

Great work, yet also limited

Hofstede's Culture and Organizations is a classic in cultural literature and is definitively worth reading. It contains numerous insights which are interesting yet also practical. A must read for anyone interested in national cultural differences.

The book consist of about 400 pages and is fairly dense in text. It consists of three different parts. The first part describes Hofstede's famous dimensions of national culture, the second discusses cultures in organizations and lasts the implications of all the earlier chapters.

In this book, Hofstede makes a lot of generalizations. I found it sometimes difficult to accept these and put my mind to "in general, statistically x is true" instead of "all people from population x are so and so." Especially when reading a book like this, it is important to keep in mind that generalizations are... exactly that. They are statistically true, but not per definition true.

Part 1 summarizes Hofstede's long research in cultural differences. His work started many years ago when he did cultural surveys in IBM and started to look at different cultural dimensions. He discovered four dimensions which he called: 1) power distance, 2) individualism/collectivism, 3) masculinity/femininity, 4) uncertainty avoidance. After a couple of years, working with a professor from the Hong Kong university, he discovered a fifth category which he called: long-term/short-term orientation. Each of these dimensions has its own chapter in which he shows the results of all countries on this particular dimension. After which he discusses how this influences the individual, family, organizations and state.

Part 1 was very insightful, yet at times the author seemed to be a victim of confusing causes and effects. At points he drew conclusions which were arguable. For example, he equated having a detached house with individualism, yet earlier he showed that individualism correlated with wealth. He leaves open whether wealth correlates to detached houses or individualism. Several times, the author jumped to conclusions where I felt he shouldn't yet. (perhaps part of his Dutch culture :P)

Part two discusses culture an organizations. The first chapter (chapter 7) argues that management theories are always limited to the culture of the creator. He makes a convincing case about this, yet I felt he went a little too far. When reading this, he seemed to argue that the management theories from one culture *never* apply to other cultures, where I'd expect a little more caution in the conclusions (similar comment as above). He especially seems to enjoy criticizing American management theories :) The second chapter in this part talks about organizational cultures. It looks at difference practice dimensions that can be identified in different organizational cultures.

Part three consist of two concluding chapters of which the first discusses intercultural encounters and culture shocks. I enjoyed how the clarifications of how different cultures encounter other cultured (ethnocentrism/xenophilia). The final chapter consist of suggestions to the world from the author. Suggestions on how to take culture into account at work, family, in the press, etc.

Cultures and Organizations is a very insightful work on cultural differences, yet I also found it limiting? Howcome? I noticed that the author especially focuses on differences in cultures in Europe and between Europe/US, yet rarely had good insight in Asian, Latin American, or African cultures. He does mention them every now and then, but still, almost all detailed examples and explanation are from Europe. Of course, this reflects the authors own experiences, but because of that (and because I've been living in Asia for many years) I felt his comments on Asian cultures was every now and then not subtle enough, which annoyed me.

Another point on which I disagreed with the author was the speed of cultural change. Hofstede is of opinion that culture does *not* change or that it takes a very very long time before it changes (relative to each other). I've lived in China for a couple years and frequently discussed cultural changes there, I think everyone will say that cultural *values* (not just practices) have changed a lot over the last 20 years. For example, on Hofstede's individualism/collectivism dimension, China was the top on collectivism. Based on my experiences, I wouldn't believe that anymore. Yes, this was true perhaps when he did the IBM survey, but not now. Similarly, Singapore scored lowest on uncertainty avoidance, which according to his explanation, shouldn't be as Singapore is the country with perhaps the most strict laws of all countries. My experience is that cultural values are changing, whereas Hofstede feels they are not (and they are perhaps even diverging more). Of course, only the future will tell us which is true, from a cultural perspective, Hofstede only surveyed a very small time-frame :)

All the comments aside, this book is definitively worth reading. I'd rate it 4.5 stars (due to some of the quick conclusions made) and decided to go for 4 Amazon stars. Definitively worth it though.

The best all-around book for residents and practitioners.

I used the third edition of this book for my writtens and orals. I am fellowed in cardiothoracic anesthesia, and boarded in pain medicine. It is more than comprehensive enough for everday practice, and it is more than enough to get you through the written boards, and will certainly help with the orals. Miller, Barash (is there an etc to these two choices?) are much more comprehensive, but they are highly dependent upon the writers of each chapter: while some are great reading, others ramble on incessantly about studies which show effects on giant squid axons, or how 3-74% of rats will react when their tails are clamped. And who can forget the effects of isoflurane on the livers of salamanders? Morgan-Mikhail is put together logically, it is highly readable, and it is extremely affordable, and while it won't give you every answer, it's a good start.

Excellent!

This would be an optimal choice for most medical students with a special interest in anasthesia as well as for anaesthesia residents. It's extremely well-written, neither too detailed nor too simple. The authors write with authority, which is a good thing.

If I had to say something negative about this book, it would be the lack of colourful illustrations (this refers to edition elder to the 2005 edition). But then again, one knows what to expect when buying a Lange medical book. Still, this is a minor drawback, and means nothing in my eyes. I highly recommend this book!

Clinical Anesthesiology

It is a good basic book that covers the essentilas very clearly in not too long chapters. It is a good entry level book for doctors during their first year of residence in anesthesia.

Also for the doctor that needs to update his knowledge.

My personal experience and the reason for buying the book was that I after 5 years i neuroanesthesia needed an update prior to a period as an anesthesiologist in a not specialised department. In this way the book served its purpose well.

Does not work in the real world of Alzheimer's Disease

I sent this book back, it could encourage unproductive activity situations. Although there are some good activities mentioned in this book, most of the activities outlined either simply don't work or are, at worst, counter productive for Alzheimer's persons. The author, who appears to lack knowledge of dementia and activities (she is a nurse not a RT) should become acquainted with Reisberg's 7 Stages of Alzheimer's or Johnson and Johnson's Alzheimer's as a 'trip back in time' article (American J. of Alzheimer's Disease, April, 2000). Just because a person works at a facility which cares for demented persons does not mean she knows what she is talking about. I wonder if this person has a degree in Gerontological Nursing or Gerontology? This author ill-advises readers to use activities that aid in "reality orientation" or "24 hour reality orientation" for a person with dementia. That idea is so dated and counter-productive it is ridiculous. The requirement for 24 hour reality orientation in nursing facilities is currently being either changed or questioned by most gerontologists. For example, the trend is for changes in the 24 hour RO requirement written into the policy and procedures for dedicated dementia wings or for nursing facilies (since they are running over 60 percent dementia),where the Activity Director validates the Alzheimer's person's delusions. Why? Because RO simply doesn't work or delievers a bad message for most demented persons. Unlike depression or delirium, demented persons have lost their abilities to think, remember and reason. So instead person-centered activities are the trend, where the activity accomopanies the AD person on his or her "trip back in time" instead of imposing your reality on the different reality of the AD person. When the Alzheimer's person has time-traveled back to age 20 or 30 and she is told she has grandchildren or an 80 year old husband, the only result of this correction will be either an argument or a message to the AD person that she is dumb! In summary, reality orientation does not work for dementia persons and should be restricted to depressed or mildly confused elders only. RO was originally used for Post Traumatic Stress Syndrome men in Alabama and then became a national requirement for all nursing homes. But nursing homes house mainly demented persons now. So this outdated procedure is counter productive for demented persons. Activities focused on person-centered care are the current trend. Person-centered activities do mostly the opposite of what is suggested in this really bad book. Person-centered activities use validation instead of reality orientation and are matched with the 7 stages/sub-stages of AD persons in sink with the person's "trip back in time."

this is the best book I have ever ead that explains alzheimers to the lay person. You know how that person feels when you finsih it and also itis obvious that the writer understand how the caregiver feels. It has enhaced the time i spend with my grandmother more than anythinf I have ever found. I would recommend this book to eveyone, it is an invaluable asset.

About the Author
Ernest Burden is an architect, writer, and principal of Burden Associates, a New York-based media and marketing consulting firm. He is the author of many acclaimed books on architecture and preservation, including The Illustrated Dictionary of Architecture, Second Edition; Illustrated Dictionary of Architectural Preservation, and Entourage: A Tracing File for Architectural and Interior Design Drawing. Mr. Burden is a frequent speaker at conventions of the American Institute of Architects, the Restoration and Renovation Conference, Build Boston, and the Society for Marketing Professional Services. He has also conducted workshops on marketing and presentation techniques for chapters of the American Institute of Architects and the Association of General Contractors.
